Student Forms


Please complete the items listed below in each step BEFORE Orientation Day at your APEX Site:

STEP 1:  Download the APEX Family Handbook and review it with your family

STEP 2:  Complete the following online forms

Tell Us About your Child  (Optional)

STEP 3:  Print the following form and drop itoff at your site on Orientation Day

Emergency Pick-Up and Health Formone per student, must be complete each year

NOTE – Please only include health conditions that would affect your child AT APEX. Examples of things NOT to include: “allergic to penicillin”, “allergic to horses”, “takes Zyrtec in the morning at home for seasonal allergies ” etc.  Examples of things TO include:  asthma, diabetes, food allergies, seizures, recurring health problems, etc.

STEP 4: Sign up for ITBS Testing (OPTIONAL)

All you need to do to sign up for Iowa Testing is pay the fee on RevTrack in step 5 below. The fee will be listed as “optional” for all students in grades 3-8 (Please Note – ITBS testing is NOT available to students in grade K-2 or 9-12. We were unable to offer the optional fee to only specific grades so all students will see it but ITBS testing is only available to grades 3-8. Students in Grades K-2 and 9-12 will not be eligible to test so please ignore the fee if it shows up on your account.)

To sign your student up, simply pay the optional fee  and your done! If you do not wish to sign your student up, leave the fee blank. Students registering to test after the December 1st deadline will be assessed an additional $10 late fee.


APEX offers free PSAT 8/9 (spring), PSAT10(spring), PSAT/NMSQT(fall), and SAT(spring) testing for the following grades. Students do NOT need to sign up for these tests now.

  • 9th grade students are encouraged to take the PSAT 8/9 instead of the ITBS
  • 10th grade students are encouraged to take the PSAT10 instead of the ITBS
  • 11th grade students are encouraged to take the SAT and PSAT/NMSQT instead of the ITBS
  • ITBS testing is not available for seniors
STEP 5: Pay your student fees – all fees due by September 30th!

NOTE about student fees – APEX provides fee assistance for those families that qualify for free or reduced lunch. We are also more than happy to arrange a payment plan for you if needed. Contact the program coordinator for assistance. Fees can also be paid by cash or check at your APEX site starting on the first day of school


NOTE for new families – you will need to activate your Infinite Campus Portal account before you will be able to log in and pay student fees. A portal activation key will be sent to you by email.

STEP 6: Sign up for volunteer duties

STEP 7: Way to Go Carpool Connections OPT OUT

APEX is working with Way To Go to help families who want to connect with others in their area for carpooling. All APEX families will automatically be enrolled with Way To Go unless you opt out. If you need help getting your kids to and from school, Way to Go can help you get started with Schoolpool for FREE. Their secure system will connect you with families in your neighborhood to share in the responsibilities of getting the kids to school and back via carpooling, walking, biking or riding RTD together. Thousands of other families are already connected….give it a try!

STEP 8: Purchase your community school supplies and bring them on the first day of classes at your site
